Description
HMG-CoA reductase serves as the rate-limiting enzyme in cholesterol biosynthesis. Its activity is controlled by a negative feedback mechanism mediated by sterols and non-sterol metabolites derived from mevalonate, the catalytic product of this enzyme. In normal mammalian cells, this enzyme is inhibited by cholesterol obtained from the uptake and degradation of low-density lipoprotein (LDL) through LDL receptors. Competitive inhibitors targeting HMG-CoA reductase upregulate hepatic LDL receptor expression, thereby accelerating the breakdown of plasma LDL and reducing circulating cholesterol levels — a key risk factor for atherosclerosis. Alternatively spliced transcript variants of the HMGCR gene have been identified, encoding distinct protein isoforms.

Application

HMG-CoA reductase catalyzes the reduction of HMG-CoA into mevalonate. Mevalonate acts as the precursor of numerous non-steroidal isoprenoid compounds and can be converted into cholesterol via a cascade of enzymatic reactions. The reaction catalyzed by HMG-CoA reductase represents the rate-limiting step of cholesterol biosynthesis. Elevated cholesterol levels in vivo contribute to a variety of cardiovascular disorders including hypertension, coronary heart disease and atherosclerosis. Inhibition of HMG-CoA reductase effectively reduces serum cholesterol concentrations and lowers the risk of cardiovascular diseases. Therefore, HMG-CoA reductase is established as a vital drug target regulating cholesterol metabolism. Statins, widely used in clinical practice, are potent HMG-CoA reductase inhibitors. Commonly prescribed cholesterol-lowering statins include pravastatin, lovastatin and simvastatin, which play a critical role in the prevention of cardiovascular diseases.
