Forsythoside B, a phenylethanoid glycoside extracted from the leaves of Lamiophlomis rotata Kudo, a traditional Chinese medicinal plant used for treating inflammatory conditions and improving blood circulation. Forsythoside B demonstrates inhibitory effects on TNF-alpha, IL-6, and IκB, while also modulating NF-κB signaling.

In Vitro Study
Forsythoside B exhibits a concentration-dependent reduction in the levels of TNF-α, IL-6, and high-mobility group-box 1 protein (HMGB1) in lipopolysaccharide (LPS)-stimulated RAW264.7 cells. It also inhibits the IκB kinase (IKK) pathway and modulates the activity of nuclear factor (NF)-κB.
In Vivo Study
Intravenous injection of forsythoside B, either alone or in combination with imipenem, leads to a reduction in serum levels of TNF-α, IL-6, HMGB1, triggering receptor expressed on myeloid cells (TREM-1), and endotoxin. Additionally, it upregulates the serum level of IL-10 while reducing myeloperoxidase (MPO) levels in the lung, liver, and small intestine.
At doses exceeding 8 mg/kg, forsythoside B demonstrates significant neuroprotective potential in rats subjected to cerebral ischemia and reperfusion. Even when administered with delays of 1 hour, 3 hours, or 5 hours after cerebral ischemia and reperfusion, forsythoside B at a dosage of 20 mg/kg exhibits noteworthy neuroprotective activity. This is evidenced by its ability to attenuate histopathological damage, including a reduction in brain infarct size, brain edema, cerebral Evans blue extravasation, and myeloperoxidase activity, as well as inhibition of cerebral phosphor-IκB-α and NF-κB expression.
Moreover, forsythoside B demonstrates significant improvement in myocardial function, characterized by enhanced left ventricular systolic pressure (LVSP) and +/- dp/dt(max). It reduces myocardial infarct volume, serum levels of Tn-T, TNF-alpha, and IL-6, as well as the content of malondialdehyde (MDA) and MPO activity in myocardial tissue. Additionally, it downregulates protein expression of HMGB1, phosphor-IκB-α, and phosphor-NF-κB, while attenuating the decrease in superoxide dismutase (SOD) and glutathione peroxidase (GPx) activities.
