Aprotinin is a serine protease inhibitor with inhibitory effects on pancreatic trypsin, pancreatic kallikrein, plasmin, and other serine proteases. It demonstrates a Kd of 0.06pM and 9.5nM for pancreatic trypsin and pancreatic kallikrein, respectively, and is utilized to reduce bleeding and transfusion during the perioperative period. Acting within the protease signaling pathway, it targets thrombin, trypsin, kallikrein, chymotrypsin, and trypsinogen. In vitro studies reveal that Aprotinin effectively inhibits fibrinolysis and prevents protein degradation during tissue and cell homogenization and lysis. It exhibits dose-dependent inhibition of fibrinolysis and prolongs clotting time. Aprotinin also demonstrates significant effects in vivo, inhibiting clot dissolution, prolonging tail bleeding time in rats, and extending clotting time in human plasma. Notably, it reduces thrombus weight in a rat arteriovenous shunt model.
